2008 - 2009 Registration is here!
Be sure to register online with USA Hockey BEFORE turning in your registration. Be sure to sign up for a PIN number on the volunteers page BEFORE completing your registration. Traveling registration dealine was September 1st. The September 23rd in person registration is for girls U8/Mite players only. If you are a traveling player, please get your registration to Heidi Keller prior to the September 21st parent tryout meetings. IN-PERSON REGISTRATION DATES
Tuesday, September 23 – 5:00 – 6:00
ALL PLAYERS:
USA on-line registration must be completed prior
to registering with CRYHA!!
Confirmation sheet must be included with your forms
Directions for on-line registration with USA Hockey:
1. Go to www.usahockeyregistration.com.
2. Choose member type “Ice Player.”
3. Check the box that acknowledges that YOU are 18 or older (person completing the registration must be 18 years of age).
4. Select the correct season – “2008-2009.”
5. Follow the prompts – fill in all data correctly – Entry your child’s name EXACTLY as it appears on their BIRTH CERTIFICATE.
6. Process payment – Visa, Master Card, Discover or American Express (credit or debit card). A Visa gift card will also work.
7. PRINT your confirmation page. You MUST include this with your registration forms. It should include a BAR CODE that will be scanned.
Each player will pay the USA Hockey fee of $30 and a MN Affiliate Hockey fee of $10. Any child born January 1, 2002 or later does not pay either fee – but you must still register with USA Hockey prior to registering with CRYHA.
Your confirmation receipt also includes the USA Hockey Waiver of Liability (which you agreed to while registering on-line.)
